General Price List
The goods and services shown within are those we can
provide to our customers. Effective Date January 1, 2009
YOU MAY CHOOSE ONLY THE ITEMS YOU DESIRE.
However, any funeral
arrangements you select will include
a charge for our services. If legal or other requirements
mean you must buy items you did not specifically ask for,
we will explain the reason in writing on the statement we
provide describing the funeral goods and services you
selected. It is the policy of our firm to require
embalming where funerals are selected
that involve viewing of the body.

“Prior to drafting any
contract for goods and services, the responsible party or the decedent's
survivor who is handling the funeral arrangements is entitled to receive a
copy of any pre-need agreement in the possession of the funeral establishment
that has been signed and paid for, in full or in part, by or on behalf of
the decedent.”

Basic Services of the Funeral Director and Staff |
$1,295.00 |
|
This
is the basic fee that we charge every family for our professional expertise
in assisting them with their preferences. Our charge includes, but is not
limited to, consultation with the family, administrative and clerical services,
preparation of the death certificate for certification by the physician or
the coroner, necessary permits, filing of the death certificate with the
County Health Department, coordination with the crematory, cemetery or
common carrier. Assistance in planning a memorial service and placement of
death notice in the newspapers selected by the family. 24-hour coverage.
Minimum care of remains (wrapping in sheet and placing in container or
selected casket). Assistance with insurance papers, notifying Social
Security department and Veteran's Administration, when applicable.
THIS
FEE FOR OUR SERVICES WILL BE ADDED TO THE TOTAL COST OF THE FUNERAL
ARRANGEMENTS YOU SELECT. (THIS FEE IS ALREADY INCLUDED IN OUR CHARGE FOR
DIRECT CREMATION, IMMEDIATE BURIAL, FORWARDING OR RECEIVING OF THE REMAINS
AND IN THE GROUPED OR PACKEGED SERVICES.) |

EXCEPT
IN SPECIAL CASES, EMBALMING IS NOT REQUIRED BY LAW.
Embalming may be necessary, however, if you select certain funeral arrangements,
such as a funeral with viewing. If you do not want embalming, you usually
have the right to choose an arrangement that does not require you to pay
for it, such as Direct Cremation or Immediate Burial. |

transportation from a common carrier to the funeral home, and
transportation of remains to cemetery or crematory. |

This
charge includes 24-hour transfer of the decedent to the funeral home from
the place of death within a 15-mile radius, basic services of funeral
director and staff, up to 72 hours of refrigeration, transportation to and
from our crematory and the crematory fee.
If you wish to arrange a direct cremation, you may use an alternative
container. Alternative containers encase the body and can be made of
materials such as fiberboard or composition materials (with or without an
outside covering). The alternative containers we provide are cardboard and
particle board. |

charges for an immediate burial include transfer of remains to our funeral

15-mile radius, at a time of our choosing and family members are not
present at gravesite. |
