Proctor Form Contract

We provide a service for ARC Professors to accommodate the needs of the DSPS students. Your DSPS student will first present you with a Proctor Form for your guidelines and signature. Please complete the back side of the Proctor Form completely. Incomplete Proctor Forms will not be accepted. The in-class time is critical to calculate the students "allowable time" for the exam. Always initial (rather than a check mark) accommodations.

All exams must include each student’s name.

It is important that you list a contact number on the proctor form.

There are times when we need to call you regarding an "unsent" exam. Also, the student may feel that there was a verbal "ok" to use other items that were not originally agreed to use, on the contract. With online exams, the program may not be accessible to the student at the time they scheduled their exam and only the professor would know that. There could be a number of reasons.

DSPS Exam Envelopes

We provide envelopes for Professors or students who are delivering exams. It is very important that you complete the information portion listed on the envelope. All tests sent with students must be sealed.

DSPS does not deliver exams

Either the student can deliver the exam to the area department, or, take the exam to the next class session. The exam will be properly sealed. The Professor can also pick up the exam during service hours.

HOW TO EMAIL EXAMS:

Email to adsps-proctor@arc.losrios.edu . A Proctor is always at this email address during office hours to print and file your exams. Do not print to DSPS email or DSPS-ALL email. Should you do that, the email will be delivered to all DSPS employees of the Los Rios Community College District. Also, email exams one day prior to the date of the exam. Direct drop-off is preferred.

Important:

Type all exam instructions for the student on the first paragraph of the first page. If an error needs to be changed, you the Professor will need to bring the changed copies to the DSPS Testing area if there is more than one DSPS student taking the exam.

If you sense that a student may benefit from DSPS, please send them to the DSPS check-in counter to schedule an appointment with a DSPS counselor.
